Based on a tour of the facility and interview it was determined that facility failed ensure that preventative maintenance programs ensure the premises were clean and orderly.

Findings were:

During a tour of the facility on 7/30/14, the following observations were made:
? In the laboratory area, a hole in the ceiling panels approximately 5 inches wide with wires going through it was observed. This open ceiling tile presents a potential entry for dust, debris, and vermin.
? In the inpatient medication room, a hole in the ceiling panels approximately 3 inches wide with wires going through it was observed. This open ceiling tile presents a potential entry for dust, debris, and vermin.
? In the kitchen, on the counter area worn areas in the laminate were observed with the plywood underneath exposed. The exposed plywood indicated the the laminate counter space cannot be effectively clean and sanitized.
? In Patient Room # 236, a ceiling tile was observed in the bathroom with water damage present approximately 6 inches in circumference. The presence of water damage indicated the area could be contaminated with bacterial particles due to the moist environment.

The above findings were confirmed in an interview on 7/30/14 with staff member # 16.
