Based on staff interview, record review, and policy and procedure review the Critical Access Hospital (CAH) failed to ensure that documentation of signed consent forms were completed for Swing Bed patients in 5 of 15 records reviewed (patients # 6, 7, 8, 9 and 10). This deficient practice puts all patients at risk for not having a voice in their care and treatment while a swing bed patient at the CAH.
Findings include:
- Record review in the conference room on 9/6/17 for patients # 6, 7, 8, 9, and 10 revealed patients admitted/transferred to Swing Bed services lacked documentation of a signed consent form.
Staff E, Administration interviewed in the conference room on 9/6/17 at 1:36 PM verified the medical record failed to provide documentation of a Swing Bed transfer/admission consent form for patients # 6, 7, 8, 9 and 10. Staff E explained the patients admitted to the CAH sign one consent form upon admission. If a patient transfers from inpatient to swing bed care the facility fails to have a separate consent form.
Staff A and B, Administration interviewed in the conference room on 9/6/17 at 4:00 PM verified the facility failed to have a separate consent form for patients admitted/transferred to the Swing Bed unit.
- The CAH failed to have an admission/consent form policy and procedure for swing bed patients.
