Based on observation during the life safety survey completed on 08/29/12, the hospital did not maintain building construction to resist the passage of smoke.

Findings:

Three approximately one inch unsealed penetrations were observed in the upper wall area of the lower level AV Room.

Based on observation during the life safety survey completed on 08/29/12, the hospital did not locate electrical light switches to be at least 60" from the floor surface as required per NFPA 99

Findings:

The electrical light switch located inside of the medical gas manifold supply room was observed to be installed at 44" above the floor surface.

This observation was made in the presence of the Hospital Director of Maintenance.

Based on observation during the life safety survey completed on 08/29/12, the hospital did not maintain electrical power cords in accordance with NFPA 70, The National Electrical Code.

Findings:

1. An extension cord was observed to be connected to a relocatable power tap in the physician lounge.

2. The flexible power cord that energized the ice machine was observed to pass through an unprotected rough cut opening in a wood cabinet where the chord may be subject to damage.

These observations were made in the presence of the Hospital Director of Maintenance.
