Based on record review and interview the facility failed to ensure that the implementation and use of video/audio monitoring was included in the patient's treatment plan in 10 of 10 medical records reviewed (Patient (Pt) 1, 2, 3, 4, 5, 6, 7, 8, 9, 10) in a total sample of 10 records reviewed with patients on video monitoring.

Findings Include:

Review of Policy and Procedure titled, "Use of Video Cameras For Patient Monitoring On Inpatient Units" last revised 02/2021 revealed the following:
1. The use of audio/video monitoring designated rooms and is based on patient treatment needs.
2. The use of video monitoring is recorded on the patient's treatment plan and updated as necessary.
3. Video/audio monitoring will be documented in the patient's treatment plan and describe the purpose of monitoring.

Review of Pt #1's medical record revealed that Pt #1 (11 years old) was admitted to the psychiatric facility on 01/31/2022 with a diagnosis of Bipolar Disorder and Suicidal Ideation with a plan. Per review of Pt #1's orders, Pt #1 was placed on video monitoring on 02/04/2022 at 2:49 pm and the order was in effect until discharge date of 02/09/2022 at 2:51 pm. Review of Pt #1's medical records revealed that video/audio monitoring was not addressed in Pt #1's treatment plan with a description of the purpose for the monitoring per facility policy.

Review of Pt #2's medical record revealed that Pt #2 (12 years old) was admitted to the psychiatric facility on 02/22/2022 with the diagnosis of Major Depressive Disorder, Anxiety, and Suicidal Ideation. Per review of Pt #2's orders, Pt #2 was placed on video monitoring on 02/22/2022 at 5:25 pm and the order was in effect until discharge date of 02/26/2022 at 2:37 pm. Review of Pt #2's medical records revealed that video/audio monitoring was not addressed in Pt #2's treatment plan with a description of the purpose for the monitoring per facility policy.

Review of Pt #3's medical record revealed that Pt #3 (13 years old) was admitted to the psychiatric facility on 02/27/2022 with a diagnosis of Major Depressive Disorder with Psychosis and Suicidal Ideation. Per review of Pt #3's orders, Pt #3 was placed on video monitoring on 02/27/2022 at 9:05 pm and the order was in effect until 03/04/2022 11:12 am. Review of Pt #3's medical records revealed that video/audio monitoring was not addressed in Pt #3's treatment plan with a description of the purpose for the monitoring per facility policy.

Review of Pt #4's medical record revealed that Pt #4 (14 years old) was admitted to the psychiatric facility on 03/07/2022 with the diagnosis of Unspecified Mood Disorder with Psychosis and Suicidal Ideation. Per review of Pt #4's orders, Pt #4 was placed on video monitoring on 03/09/2022 at 8:29 pm and the order was in effect until discharge date of 03/14/2022 at 9:30 am. Review of Pt #4's medical records revealed that video/audio monitoring was not addressed in Pt #4's treatment plan with a description of the purpose for the monitoring per facility policy.

Review of Pt #5's medical record revealed that Pt #5 (12 years old) was admitted to the psychiatric facility on 03/21/2022 with the diagnosis of Major Depressive Disorder Severe and Obsessive Compulsive Disorder. Per review of Pt #5's orders, Pt #5 was placed on video monitoring on 03/21/2022 at 7:24 pm and the order was in effect until 03/23/2022 at 1:30 pm. Review of Pt #5's medical records revealed that video/audio monitoring was not addressed in Pt #5's treatment plan with a description of the purpose for the monitoring per facility policy

Review of Pt #6's medical record revealed that Pt #6 (10 years old) was admitted to the psychiatric facility on 03/15/2022 with the diagnosis of Major Depressive Disorder Severe, Eating Disorder, and Self Harm. Per review of Pt #6's orders, Pt #6 was placed on video monitoring on 03/19/2022 at 11:23 am and the order was in effect until 03/23/2022 at 1:25 pm. Review of Pt #6's medical records revealed that video/audio monitoring was not addressed in Pt #6's treatment plan with a description of the purpose for the monitoring per facility policy

Review of Pt #7's medical record revealed that Pt #7 (15 years old) was admitted to the psychiatric facility on 03/23/2022 with the diagnosis of Major Depressive Disorder Severe and Obsessive Compulsive Disorder. Per review of Pt #7's orders, Pt #7 was placed on video monitoring on 03/23/2022 at 8:33 pm and the order was in effect until discharge date of 03/28/2022 at 2:45 pm. Review of Pt #7's medical records revealed that video/audio monitoring was not addressed in Pt #7's treatment plan with a description of the purpose for the monitoring per facility policy.

Review of Pt #8's medical record revealed that Pt #8 (12 years old) was admitted to the psychiatric facility on 04/11/2022 with the diagnosis of Psychosis, Anxiety, and Eating Disorder. Per review of Pt #8's orders, Pt #8 was placed on video monitoring on 04/11/2022 at 8:46 pm and the order was in effect until 04/14/2022 at 1:21 pm. Review of Pt #8's medical records revealed that video/audio monitoring was not addressed in Pt #8's treatment plan with a description of the purpose for the monitoring per facility policy.

Review of Pt #9's medical record revealed that Pt #9 (13 years old) was admitted to the psychiatric facility on 04/27/2022 with the diagnosis of Major Depressive Disorder Severe, Obsessive Compulsive Disorder, and Post Traumatic Stress Disorder. Per review of Pt #9's orders, Pt #9 was placed on video monitoring on 04/27/2022 at 12:51 pm and the order was still in effect at the time of medical record review on 05/04/2022 at 10:55 am. Review of Pt #9's medical records revealed that video/audio monitoring was not addressed in Pt #9's treatment plan with a description of the purpose for the monitoring per facility policy.

Review of Pt #10's medical record revealed that Pt #10 (13 years old) was admitted to the psychiatric facility on 04/25/2022 with the diagnosis of Major Depressive Disorder Severe and Post Traumatic Stress Disorder. Per review of Pt #10's orders, Pt #10 was placed on video monitoring on 05/02/2022 at 9:31 pm and the order was still in effect at the time of medical record review on 05/04/2022 at 11:15 am. Review of Pt #10's medical records revealed that video/audio monitoring was not addressed in Pt #10's treatment plan with a description of the purpose for the monitoring per facility policy.

Per interview with Team Leader B on 05/04/2022 at 10:00 am, "B" stated that staff do not typically document and update video monitoring in the treatment plan. Per Team Leader B, this is not "necessarily" communicated to the nursing staff to include.
