During the survey tour of 06/23/2015 at 1045, while accompanied by the Plant Operations Manager, through observation and staff interview, it was noted that the kitchen hood tags were not current (fire suppression system and cleaning). At 1530, during record review with the P.O.M., current hood testing documents could not be produced. A telephone call to the vendor(s) confirmed that the facility was overlooked for servicing and is out of compliance with NFPA 96. This could result in failure of the kitchen hood suppression system to function properly, putting staff, visitors and patients at risk. This finding was acknowledged by the Plant Operations Manager.

The deficiencies are as follows:

1. The most current tag on the UL300 system is punched-out 11/2014 and the last document from Simplex is dated 11/11/2014. There is neither evidence nor knowledge of required semi-annual testing since that date.
2. The hood cleaning tag is dated 03/2014. There is neither evidence nor knowledge of required annual service since that date.

The Plant Operations Manager stated that he was unaware that the service had not been performed by the vendor.
