Based on observation, staff interviews, review of CDC (Centers for Disease Control and Prevention) guidance, and review of facility policy and procedure, it was determined that the facility failed to ensure that infection control practices used to mitigate the spread of COVID-19, are implemented.

Findings include:

Reference #1: Centers for Disease Control and Prevention (CDC), Interim Infection Prevention and Control Recommendations for Healthcare Personnel During the Coronavirus Disease 2019 (COVID-19) Pandemic, https://www.cdc.gov/coronavirus/2019-ncov/hcp/infection-control-recommendations.html, updated July 15, 2020 states, "CDC recommends using additional infection prevention and control practices during the COVID-19 pandemic... Implement Universal Source Control Measures... Source control refers to use of cloth face coverings or facemasks to cover a person's mouth and nose to prevent spread of respiratory secretions when they are talking, sneezing, or coughing. ... HCP (healthcare professionals) should wear a facemask at all times while they are in the healthcare facility, including in breakrooms or other spaces where they might encounter co-workers. ... ."

Reference #2: Facility policy, "COVID-19 (2019-nCoV) Persons of Interest: Identifying, Isolating, and Reporting Interim Policy" states, "1. Patient entry points should have posted information instructing patients on the following ... c. Relevant travel and exposure history that should be reported to the registration clerk. 2. Registration staff should ... d. Inquire about travel or exposure history... e. Immediately contact the triage nurse if a patient identifies as having a significant travel or exposure history ... ."

1. During a facility tour on 9/29/20 at 10:55 AM, the following was observed:

a. At the security desk in the Main Entrance lobby, Staff #6 and Staff #7 were sitting in close proximity to one another while engaged in a conversation. Staff #6 and Staff #7 were wearing their facemasks below their nares.

b. Staff #5 entered the lobby wearing his/her facemask below his/her nares.

c. During the screening process at the Main Entrance, Staff #10 did not ask visitors about travel history, as indicated in facility policy.

(i) There were no signs posted containing information regarding recent travel, as indicated in facility policy.

2. Staff #1 and Staff #4 confirmed the above findings.
