The hospital failed 2 of 2 Patient's (Patients #1 and #2), by failing to provide care in a safe setting.
Findings Include
During interview with Staff #1, It was discovered that Patient #1 and Patient #2 were found in the dayroom of the hospital engaged in vaginal sex.
During interiew with Staff #4, indicated staff remaining on the unit were monitoring a new patient determing whether or not to call a code on the patient. The other staff was completing meal preparation.
During interview with Staff #3, Staff #3 and Staff #4 had to physically separate Patient #1 and Patient #2 from engaging in phyiscal vaginal sex.
During interview with Staff #1, and Staff #2, the hospital is having a difficult time keeping quality hospital staff working."The hospital is short staffed."
During interview with hospital Staff #1 and hospital Staff #4, there were no remaining staff to monitor the remaining patients on the unit as all staff were in observation of the agitated patient.
Policy
The hospital Policy on Patient Rights dated 04/2018, reflected, "Is the policy of Millwood Hospital and Excel Centers to ensure that all patients receive a copy of the Patient Handbook which indicates the Patient Bill of Rights as well as an oral explanation of those rights both in their primary language and in simple non-technical terms. We will strive to abide by and respect all patient rights without regard to race, religion, creed ethnicity, gender, age, sexual orientation, or handicap. Millwood Hospital and Excel Centers shall support and protect the fundamental human, civil constitutional and statutory rights of the individual patient and recognize and respect personal dignity of the patient at all times."
The hospital Policy on Patient Abuse dated 03/2018, reflected, "To establish guidelines and procedures for the reporting of child, elderly or disabled persons abuse in accordance with the Texas Penal Code. Reports are made if a person has cause to believe that the physical or mental health or welfare has been or may be adversely affected by abuse or neglect."
The hospital Policy on Patient Observation Rounds dated 06/2018 reflected, "To ensure patient safety, as well as, to provide process for observing and documenting patient location and behavior."
