Based on staff interview and the review of facility policy, it was determined that the facility failed to include criteria for the assessment of pain in the pain assessment policy.

Findings include:

Reference: Facility policy titled, "Pain Assessment and Management of the Adult" states, " ...All patients who screen positive for pain will have a pain assessment conducted, which will include severity of pain and location ....."

1. Upon request, Staff #3 provided a copy of the facility policy, titled, "Pain Assessment and Management of the Adult" to be reviewed. The policy did not include the following criteria for the assessment of pain: pain character, pain frequency or pattern, pain duration, precipitating factors, responses to treatment and the personal, cultural, spiritual, and/or ethnic beliefs that may impact and individual's perception of pain.
