Based on record review, and interview, the hospital failed to ensure patient received care in a safe setting for 1 of 10 patients.
This failed practice resulted in physical harm and injury, thereby impeding on a patient's right to care in a safe setting.
Findings:
On 12/29/21 Patient #1 self-inflicted 2 lacerations with a disposable razor resulting in 21 staples to left forearm.
Facility failed to follow policy and procedure outlined in a document titled "Use of Razors."
Interview and record review showed, patient #1 was permitted use of a razor and the facility did not verify the razor was returned after use.
