Based on review of facility documents, medical records (MR), and staff interviews (EMP), it was determined the facility failed to conduct an investigation of an allegation of abuse reported by a patient representative for one patient (PT1) of one of one medical records reviewed (MR1).

Findings Include:

Review on March 15, 2023 of facility brochure "Patient Rights and Responsibilities " dated 2017, revealed "... We are committed to honoring your rights ... Your Rights ... As our patient ... right to safe, respectful and dignified care at all times,,,. Care Delivery... receive care... safe setting free from abuse ... neglect ... receive ... respectful, safe, quality care ..."

Review on March 16, 2023 of facility policy "Patient Complaint and Grievance Resolution Process'" revised August 2022, revealed "... Grievances ... 5. Grievances submitted to hospital personnel in writing, verbally, via email, over the telephone, or over GVH's website are forwarded to Patient Relations and Risk Management. 6. A Patients Relations Coordinator will communicate with the patient, or their representative regarding the plan to investigate and follow-up the grievance. At that point, the investigation of the grievance shall be coordinated with he appropriate leadership staff ... 7. The hospital will strive to send a written response to all grievances within 7 calendar days of the receipt of the grievance by the Pateint Relations Department. However, in circumstances where grievances are complex and require extensive investigation, this timeline may not be possible. ... 9. Written notice of the hospital's determination regarding the grievance will be provided to the patient or the patient's representative ... 13. A copy of the response is documented and filed confidentially with the original grievance and other related documention in the Patient Relations Department."

Review on February 27, 2023 of facility document "Risk Management/Patient Safety ..." no date, revealed the facility was first aware of an allegation of abuse related to PT1 on January 29, 2023.

Interview with EMP1 on February 27, 2023 at 10:37 AM revealed the facility failed to conduct an internal investigation of the alleged report of abuse related to PT1.

Interview with EMP1 on February 28, 2023 at approximately 2:00 PM confirmed the facility failed conduct to an internal investigation of the alleged report of abuse related to PT1.
