Based on observation during the life safety survey completed on 01/28/2015, the hospital did not maintain corridor doors to be free from obstructions to closing and positive latching as required.
Findings:
The LTC manager office corridor access door was observed to be held open by a rubber floor wedge. The wedge was removed during survey.
This observation was made in the presence of the Director of Plant Operations.

The Central Supply area corridor door was observed to be held open by a floor wedge. The wedge was removed during survey.

Based on observation during the life safety survey completed on 01/28/2015, the facility did not maintain the fire sprinkler system in accordance with NFPA 13 to prevent sprinkler obstruction.
Findings:
A sprinkler head located in the mechanical room was observed to have the manufacturer provided protected sleeve installed that would prevent the head from activating as designed.
This observation was made in the presence of the Director of Plant Operations.

Based on observation during the life safety survey completed on 01/28/2015, the facility did not maintain exhaust plenums to be free from grease laden vapor build-up in accordance with NFPA 96.
Findings:
The exhaust plenum immediately above the high temp dishwasher was observed to have an excessive accumulation of grease laden dust and dirt deposited on the inside of the exhaust plenum.
This observation was made in the presence of the Director of Plant Operations.
