Based on observation, record review and interview the facility failed to maintain emergency equipment.
Findings include:
On the crash cart on the nursing floor, the suction machine was not set up for an emergency creating a risk to patient care
Review of policy entitled " Crash Cart/Defibrillator Daily check " revealed all crash carts are to be checked daily.
An interview with the chief nursing officer on 9/23/14 at 1:30pm confirmed the above findings.

Based on observation and staff interviews it was determined that the facility
failed to a safely construct the environment as evidenced by lack of patient emergency
call systems in two (2) of two (2) patient restrooms which were out of hearing
and sight of the patient care staff.

Findings were:

On 9/22/14 at 1:40 PM it was noted that the patient restroom in the
Outpatient Department,which was out of site and hearing of patient staff
had no emergency call system. In a conversation the next day with the
corporate compliance officer confirmed that there was no call light in
that patient restroom.

On 9/24/14 at 10:00 AM on tour with the director of rehabilitation services
it was noted that there was a patient restroom in the hall outside
of the department which was out of site and out of hearing. It had no
emergency call system either. The director of rehabilitation services
entered the restroom and confirmed the findings.
