LIFE SAFETY CODE STANDARD

Based upon observation and interview, it was determined the facility failed to maintain exit access requirements in one instance on one of four floors.

Findings include:

1. Observation on June 11, 2014, revealed the following exit access deficiency:

a) At 1:10 p.m., the exit corridor on the third floor outside of the CHRU had several items blocking exit access. These items included a crib, two beds, a table, two chairs, and a portable x-ray machine.

Interview with the Chief Executive Officer and Maintenance Director on June 11, 2014, at 2:45 p.m., confirmed the exit access issue.

Based upon observation, document review, and interview, it was determined the facility failed to maintain the generator in one instance affecting four of four floors.

Findings include:

1. Observation on June 11, 2014, revealed the following generator deficiency:

a) At 1:57 p.m., during document review it was revealed that the facility has not been completing weekly visual inspections since March, 2014

Interview with the Chief Executive Officer and Maintenance Director on June 11, 2014, at 2:45 p.m., confirmed the generator issue.

LIFE SAFETY CODE STANDARD

Based upon observation and interview, it was determined the facility failed to maintain electrical wiring requirements in one instance on one of four floors.

Findings include:

1. Observation on June 11, 2014, revealed the following electrical deficiency:

a) At 10:19 a.m., in the first floor isolation room 136 there was medical equipment including a bed and respirator plugged into a surge protector that was plugged into another surge protector that was plugged into the wall outlet.

Interview with the Chief Executive Officer and Maintenance Director on June 11, 2014, at 2:45 p.m., confirmed the electrical wiring issue.
