Based on observation, policy review, and staff interview, the facility failed to ensure staff adhered to infection prevention and control policies and procedures related to screening of visitors for COVID-19. This affected one of two screenings observed. The facility's active census at the time of the survey was 728.
Findings include:
Upon entering the facility on 04/19/21 at approximately 9:05 AM, and reporting to the registration desk, the staff member failed to screen this surveyor for a temperature or symptoms of COVID-19. A few minutes later two additional staff members were encountered and escorted this surveyor to a conference room. Neither of these staff members screened this surveyor for COVID-19 or inquired if a screening was completed.
Review of the facility policy titled "Interim COVID-19 Protocol," (#1950) revised 04/16/21, revealed at section 4.9.2 for visitation "Temperature and symptom screening prior to entering workplace. We will continue to use thermal scanners to obtain temperatures of visitors. Anyone with a temperature above 100.0 degrees, has any COVID-19 symptoms or has been exposed to COVID-19 will not be permitted to visit."
