Based on observation, the facility failed to maintain a hazardous area door in accordance with NFPA 101 (00), Chapter 19, Section 19.3.2.1 and 19.3.6.3.2, and Chapter 8, Section 8.2.3.2.3.2. In a fire emergency, this deficient practice could adversely affect 10 of 20 patients, staff and visitors.
FINDINGS INCLUDE:
On 08/22/2013 at 10:50 AM, observation revealed the corridor door to the Soiled Linen Room adjacent to Patient Room 118, did not self-close, as the door was not equipped with an automatic door closing device.
This finding was confirmed with the chief building engineer.

Based on observation, the facility failed to maintain an automatic fire sprinkler system in accordance with NFPA 101 (2000) Chapter 19, Section 19.3.5 and NFPA 13 (1999). In a fire emergency, this deficient practice could adversely affect 2 of 20 residents, staff and visitors.
FINDINGS INCLUDE:
On 08/22/2013 between 10:00 AM and 2:30 PM, observation revealed room divider curtains installed in Resident Rooms 160 and 170, were not manufactured with the required 1/2-inch minimum diagonal mesh at the tops of the curtains. As such, these non-conforming curtains could create obstructions to the full development of the fire sprinkler spray pattern.
These findings were confirmed with the chief building engineer.
