Findings:
The inspector observed, while accompanied by the CEO, during the hours of the inspection from 3:00 pm to 6:00 pm on 04/21/2023, that there were the following issues:
1) Based on review of the records, facility failed to perform inspection of doors in means of egress annually. There were no records for the year 2020 to 2023.

Findings:
The inspector observed, while accompanied by the CEO, during the hours of the inspection from 3:00 pm to 6:00 pm on 04/21/2023, that there were the following issues:
1) Based on review of the records, facility failed to perform yearly testing of hospital-grade receptacle in patient care areas.

K918 Electrical Systems - Essential Electric System Maintenance and Testing
Findings:
The inspector observed, while accompanied by the CEO, during the hours of the inspection from 3:00 pm to 6:00 pm on 04/21/2023, that there were the following issues:
1) Based on review of the records, facility failed to perform yearly inspection of the generator and load bank test.
2) Facility failed to perform four continuous hours load bank test every 36-month.
3) At the generator location, there was no light with 90-minute battery backup and a red electrical receptacle connected to the essential electric system.
