a. Based on observation on 05/10/2011 the corridor smoke door near room 254 failed to close ( dragging the floor ).

A. Based on observation on 05/10/2011 storage room #377 was not covered by a sprinkler head.

A. Based on observation on 05/10/2011 the corridor door to the linen storage room #144 failed to latch when closed.

Based on observation on May 12, 2011 at approximately 10:30 AM onward the following was noted:
1) Exit discharge lighting was not provided at the exit doors in the basement area.

A. Based on observation on 05/12/2011 the staff interviewed did not know the fire drill procedure and also had trouble finding the right key to activate/reset the Fire Alarm pull stations.

Based on observation on May 12, 2011 at approximately 10:30 AM onward the following was noted:
1) The strobe lights in building #1 on first floor east did not operate on battery back-up power when testing the fire alarm system

2) Based on observation on 05/12/2011 when the fire alarm was tested there was a strobe not working on the second floor East near room 218.

A. Based on observation on 05/11/2011 there were linen carts stored in the egress corridor on the second floor ( near room 202 )

A. Based on observation on 05/11/2011 there were incandescent light fixtures in the 1 East soiled linen room there were not protected by a cover.
