Based on observation, two (2) out of four (4) staff interviews and review of Manufacturer's Instructions for Use (IFU) conducted on 4/22/21, it was determined that the facility failed to ensure that manufacturer's instructions for use of disinfectant wipes are followed.

Findings include:

Reference #1: Diversey Virex II 256 - One Step Disinfectant Cleaner and Deodorant Manufacturer Instructions for Use states, " ... For Use as a One-Step Cleaner/Disinfectant: Pre-clean heavily soiled areas. Apply Solution to hard, non-porous environmental surfaces. All surfaces must remain wet for 10 minutes. Wipe surfaces and let air dry. ..."

Reference #2: MEDLINE MicroKILL One Germicidal Alcohol Wipes - Manufacturer Instructions for Use states, " ... DISINFECTING: To disinfect hard, non-porous surfaces, use one [1] or more wipes as necessary, to thoroughly wet the surface to be treated. Treated surface must remain visibly wet for one minute to achieve disinfection ..."

Findings include:

1. During an observation in Patient Room #106 on 4/22/21 at 10:58 AM, Staff #8 was observed wiping and cleaning the patient's bathroom sink. Staff #8 stated that he/she uses a solution of Virex to clean and disinfect patient areas. When questioned about the contact time for Virex, Staff #8 stated that he/she did not need to wait any time before wiping off the disinfectant solution.

a. This is not in accordance with the manufacturer's instructions for use which states, "... All surfaces must remain wet for 10 minutes. Wipe surfaces and let air dry. ..."

b. A review of Staff #8's education file indicated that Staff #8 had received education on cleaning and disinfecting patient areas.

2. During an interview on 4/22/21 at 11:59 AM, Staff #12 stated that he/she utilized MEDLINE MicroKILL One Germicidal Alcohol Wipes to clean and disinfect reusable equipment and surfaces. When questioned about the contact time for the wipe, Staff #12 stated that he/she only had to wipe and keep surfaces wet for 30 seconds.

a. This is not in accordance with the manufacturer's instructions for use which require surfaces, "... to remain visibly wet for one (1) minute to achieve disinfection ..."

b. A review of Staff #12's education file indicated that Staff #12 had received education on cleaning and disinfecting reusable equipment and patient areas.

3. Staff #1 confirmed the above findings.
