Based on observation and staff interview it was determined the facility failed to ensure the door leading into the room containing the controlled drug vault was kept locked in the main pharmacy.

Findings include:

During tour of the main pharmacy on 9/23/13 at approximately 10:30 a.m. the door to the room leading into the room containing the controlled substance vault was observed to be open with no staff member in the room.

During interview earlier at approximately 9:30 a.m. the pharmacy director had indicated the door is closed and locked when no one is in the room. It was noted that two electric cords were preventing the door from closing completely. The pharmacy director stated the door should be closed and locked.
