PATIENT RIGHTS: GRIEVANCES

Based on chart review and interview the facility failed to assist the patient in completing the complaint and grievance procedure in 1(#1) of 5(#1-5) chart reviews.



Review of patient #1's chart revealed patient #1 was in the patient day area cursing and being loud. One of the Mental Health Technician (MHT) attempted to redirect patient #1. Patient #1 balled her fist and attempted to strike the MHT. Patient #1 was placed in a physical hold and taken to her room. Patient #1 was released and regained control, no medication was needed. Patient #1 went to the phone and called the state hotline and made a complaint about the hold.


Review of the complaint and grievance log revealed there was no documentation of the complaint patient #1 made on 9/17/15 to the state hotline.


An interview on 9/18/15 with staff #4 and #8 revealed staff #4 was not notified of the incident or of the patient complaint. Staff #8 reported the patient did call the hotline and he was trying to assist in resolving the patients complaint. Staff #8 reported he was not sure why staff #4 was not notified of the patient complaint. Staff #4 and #8 confirmed there had been a communication breakdown and staff #4 should have been notified to complete the complaint/grievance process.

NURSING CARE PLAN

Based on record review and interview the facility failed to list a medical diagnosis of a condition that was treated at the facility.


Review of patient #1's chart revealed patient #1 was diagnosed with a Urinary Tract Infection (UTI) in the facility's Emergency department (ED) prior to admission to the psychiatric unit. Patient #1 was started on Bactrim DS 160mg every 12 hours for 10 days on 9/13/2015.


Review of patient #1's treatment plan revealed there was no documentation found of the UTI, goals, or interventions.


An interview with staff #4 (RN) confirmed there was no documentation on the treatment plan for the UTI.
