K500 Building Services - Other
Findings:
The inspector observed, while accompanied by the EOC Director, during the hours of the survey from 1 pm to 5:30 pm on 06/13/2024, that there were the following issues:
1) Based on observation in main electrical room, the peel 'n stick label for the damper was fading and needed replacement.

K711 Evacuation and Relocation Plan
Findings:
The inspector observed, while accompanied by the EOC Director, during the hours of the survey from 1 pm to 5:30 pm on 06/13/2024, that there were the following issues:
1) Based on review of the records, facility failed to provide date when the evacuation and relocation plan was last reviewed and/or revised.

K712 Fire Drills
Findings:
The inspector observed, while accompanied by the EOC Director, during the hours of the survey from 1 pm to 5:30 pm on 06/13/2024, that there were the following issues:
1) Based on review of the records, facility failed to include the transmission of a fire alarm signal as part of its required fire drill.

K911 Electrical Systems - Other
Findings:
The inspector observed, while accompanied by the EOC Director, during the hours of the survey from 1 pm to 5:30 pm on 06/13/2024, that there were the following issues:
1) Based on observation in electrical room known as "204 electrical", there was storage of combustible rubbish which needed to be removed from this room.

Findings:

1) Based on observation at the generator set location, there was a missing light bulb and cover.
