Based on visual observation the facility failed to provide 1of 2 exits that are accessible. This would allow for the exits to not be used in the case of an emergency and potentially affecting 19 of the 19 residents of the facility.
Findings:
1) - During facility tour on 06/07/2011 between the hours of 2:00pm and 4:30pm, the exit door at the north end of the hall on the 4th floor has a special locking mag lock device with a badge swipe device., the door is not in direct view of a nurses station because of the cross corridor doors between the nurses station and the exit door does not have a key override device.
