At 10:27, observed during walk thru inspection ceiling deck of outside boiler room.
At 10:29, observed during walk thru inspection ceiling deck of outside electrical room.

On September 7, 2012 at 0920 accompanied by the Maintenance Director, there was an opening in the wall that did not have a smoke damper installed.

At 10:32, observed during inspection walk thru sprinkler system riser is incorrectly installed and mounted to where it is blocking pull station by restricting view and access to activate in outside dirty linen room.

At 11:15, observed during inspection walk thru pull station is incorrectly installed and mounted greater than 5 feet from outside exit door of MRI unit.
