Based on observation, on May 7, 2014 at approximately 10:00am onward, there is no positive latching hardware on Wickett doors located in second floor Psych Unit - Women's Unit. Doors are located in patient room door openings bordering a required exit corridor - the area is not designed as a patient sleeping suite.

Based on observation, on May 7, 2014 at approximately 10:00am onward, the essential electrical system is incomplete due to the following:
1. there is no means for testing the audible signaling devices for the generator annunciator panels located in the engineering office and security station near ED.
2. there is no required signaling, audible and visual, for less than three hour fuel operating supply in the main fuel tank serving the emergency generators of the essential electrical system.

Based on observation, on May 7, 2014 at approximately 10:00am onward, the essential electrical system is incomplete due to the following:
1. there is no minimum #10 green insulated bonding jumper bonded to normal and emergency panelboards serving the same patient vicinity - electrical equipment room located on the first floor near the Spa.
2. Electrical panelboards are equipped with equipment grounding conductors smaller than #4 with green tape on ends of conductors - conductors smaller than #4 shall be continuous green insulated conductors - located in first floor electrical equipment room near Spa.
