It was observed that the fire door located in the OB area needs to be adjusted to make a proper seal to assist in preventing heat and smoke from penetrating the separation area. It was also observed that the seperation wall on the 3rd floor between the "62 & 68" wing had openings in the seperation wall that area required to be sealed.

On 05/23/2016 surveyors 16776 and 20980 and assistant Plant Manager found In the OIT closet 2nd floor by room 2339, fire sprinlker piping noticed wiring hanging and attached to piping. Fire Sprinkler piping are not to support nonsystem components. As Per Code NFPA 13.

This tag not met as observed by surveyors 20980 and 26640 and the facility engineering supervisor. At 9 am on 5-24-2016 after conducting an outbrief surveyor 26640 asked surveyor 20980 a question concerning the "red tank" located next to or in the area of the main entrance. The question concerned what the tank was and was it correctly installed. Surveyor 20980 contacted the facility engineering supervisor and discussed the tank in question. It was found that the "red tank" is going to be utilized as a temporary heating fuel tank while the normal underground storage tank is being cleaned and changed over from #5 heating oil to #2 heating oil. The temporary tank was installed by a contractor not the facility. After conferring with a person from the Maine State Fuels Board it was found that the tank is in fact incorrectly installed and can not be used until the proper installation can be made. The contractor that installed the tank was contacted and the facility engineering supervisor was instructed that the tank was not to be used until correctly installed. Tank must be connected as required by NFPA 101, NFPA 91 and applicable manufactures requirements.

This tag not met as observed by surveyors 20980 and 16776 and the facility engineering supervisor. It was observed that an electrical panel located in the attic (south west section) had openings. These openings are required to be sealed off to prevent possible electrical shock to workers.
