Based on observation, record review and staff interview it was determined the facility failed to ensure the facility was protected throughout by an approved automatic sprinkler system in accordance with National Fire Protection Association (NFPA) 13 (see Tag K 351); failed to ensure smoke barriers were constructed and maintained to the appropriate fire resistance rating in accordance with NFPA 101 (see Tag K 372); failed to ensure air-conditioning, heating, ventilating ductwork and related equipment was in accordance with NFPA 90A (see Tag K 521); failed to ensure periodic testing and inspection of elevators in accordance with NFPA 101 (see Tag K 531); failed to ensure appropriate maintenance and testing of the emergency generator and associated equipment was performed in accordance with NFPA 110 (see Tag K 918); and, failed to ensure appropriate testing and maintenance requirements for fixed and portable patient-care related electrical equipment (PCREE) in accordance with NFPA 99 (see Tag K 921).
