Based on document review and staff interview the facility failed to provide proof of testing of HVAC fire sfety devices in the required interval. Faillure to test protective devices could result in failure of the devices during a fire event, which risks safety of patients, staff and visitors.
The finding is:
On 3/15/2022 at 2:00pm, while in in the company of DOPO, it was determined that HVAC fire sfety devices are not tested in accordance with NFPA 80-2010, 19.4. The evidence presented indicated that the devices were last tested 10/05/2015, which is approximately 6 months beyond the required 6 year intervals for hospitals at the time of the survey.

During document review the facility could not provide testing of the Essential Electrical System. Failure of testing may affect patient, visitors and staff during a utility failure.
The findings are:
On 3/15/2022 at 2:00pm, while in the company of DOPO, evidence of the following could not be provided:
A. Documents for the weekly inspection of the facility emergency generator as required by NFPA 110-2010, 8.4.1 were not available for review.
B. Documents for the monthly operation of the facility emergency generator under load as required by NFPA 110-2010, 8.4.2 were not available for review.
C. Documents for an annual load bank operation and testing for the facility emergency generator covered under NFPA 110-2010, 8.4.2.3 were not provided. Evidence was provided of a load bank test is from the initial acceptance test dated 2/11/2021.
