Based on record review and interview, the facility failed to discharge the patient to an appropriately licensed Assisted Living Facility (ALF) in 1 out of 11 sampled patients (SP#1).
Findings:
Review of sampled patient (SP) #1 medical record showed that the patient (pt.) was evaluated and admitted for 24-hour observation on 10/18/16. The pt. was stabilized and was discharged on 10/19/16 to an unlicensed Assisted Living Facility (ALF). The physician discharge order dated 10/19/2016 showed in the comments: Assisted Living Facility. The Discharge Summary (hospital course) showed the CM (case manager)/ SW (social worker) consulted for help with placement-and ALF was arranged. DC (discharge to home/ALF. The Nurses Notes dated 6:59 PM showed SP #1 was discharged to the ALF. The Pt went to the ALF via ambulance.
On 12/28/16 at 11:35 AM, the AVP (Assistant Vice President) of Case Management (CM) confirmed above findings that SP#1 was discharged to above facility. She further stated, that we rely on the agency on placement and it seems that they did not do their job correctly. We need to go back to the agency to make sure that they are not providing us with an inappropriate agency.
On 12/29/16 at 10:40 AM, Social Worker (SW# A) via phone stated, I did not check the ALF license status because they said the place was licensed. I called again the placement agency today to verify if they have proof that they evaluated the ALF and they cannot provide any proof. Henceforth, I make sure that I check the website to verify that they are licensed.
The policy subject: " Discharge Planning " (reviewed/revised 12/2015), state the purpose is to ensure a smooth transition through the continuum of care; maintain continuity of care through the transition; and execute a safe, effective, and appropriate discharge plan. The policy further state that a plan must be developed to provide for the continuum of care and to address the patient ' s immediate needs following discharge.
The facility did not adhere to its policy.
