COVID-19 Vaccination of Facility Staff

Based on review of the facility's COVID-19 vaccination report, policy review and interview, the facility failed to ensure 100% of staff have received the necessary doses to complete the vaccine series (i.e., one dose of a single-dose vaccine or all doses of a multiple vaccine series) or have been granted a qualifying exemption, or are identified as having a temporary delay as recommended by the CDC, (Centers for Disease Control and Prevention). The facility's active census was 1,130.

The facility's COVID-19 Vaccine Policy (Version 3, Effective 01/17/22) was reviewed. It stated:
Definitions
Individuals - Any and all individuals who provide care, treatment, or other services to the facility or its patients. This includes, but is not limited to, all employees, all employed physicians and non-employed licensed independent providers both privileged at and/or affiliated with the facility or under contract with the facility, as well as trainees, students, volunteers, vendors/third parties/contractors, and contracted employees.
All Individuals must complete a primary COVID-19 vaccination series by 02/28/22, unless they have received approval for a medical or religious exemption as set forth below.
On 03/30/22, facility staff COVID-19 vaccination list revealed the facility had a total of 37,030 employees. There were 35,135 employees fully vaccinated for COVID-19, 24 employees partially vaccinated for COVID-19, 1,842 employees had exemptions and 29 employees who had been granted an extension indicating a staff vaccination rate of 99.9%.
The facility had 6,846 contracted staff. There were 5,099 contracted staff fully vaccinated for COVID-19 or had an approved exemption indicating a contracted staff vaccination rate of 74.4%
The findings were share with Staff I on 03/30/22 at 12:00 PM and confirmed. Staff I reported the facility has given all staff and contracted staff until 03/31/22 to provide vaccination/exemption documentation or their badge access will be deactivated and their employment will be termed on 04/01/22.
