PATIENT RIGHTS: NOTICE OF GRIEVANCE DECISION

Based on record review and interview, the facility failed to provide the patient with written notice of its decision regarding the resolution of the grievance and results of the grievance process for one (1) out of 10 sampled patients (SP). (SP#1)

Findings included:

Review of the complaint log for April, 2015 showed an incident regarding the clinical care- medical doctor was received from SP#1 by the Chief Medical Officer. Review of the Patient Relations Worksheet (#15-147 and #15-146) showed that on 4/15/15, SP#1's mother sent letter to the facility with concerns regarding her dissatisfactions with the medical services provided by NICU( Neonatal Intensive Care Unit) physician.

Review of facility Policy and Procedure regarding " Patient and Family Complaint Grievance Process " showed under Procedure #4. b. " Letter with resolution and/or investigation will be sent to family within 30 days of the grievance by department director or appropriate Vice President and will include the following: Name of Hospital B contact person, Steps taken on behalf of the patient to investigate the grievance, results of the grievance process and date of completion. "

Interview with the Director of Accreditation and Regulatory on 6/18/15 at 4:00 PM confirmed above findings that the facility failed to send a letter to SP#1 mother as stated in their policy and procedure.
