Based on observation and interview the facility failed to maintain a sanitary environment in five of five rooms (#225,#664, #665, #667, and #668) resulting in the potential for the spread of infectious disease among patients. Findings include:
On April 29, 2014 at approximately 1100 a tour of the surgical specialty unit was conducted. In room 668 the inside rails of the patient bed were found to have an accumulation of dust and debris. Staff B and staff I confirmed the findings at approximately 1105 on April 29, 2014. On April 29, 2014 at approximately 1107 the patient bathroom in room 668 was observed to have dirty tiles behind the shower curtain and dirty tiles around the bathroom sink. Staff B and staff I confirmed the findings on April 29, 2014 at approximately 1110.
On April 29, 2014 at approximately 1112 in room 667 of the surgical specialty unit the inside rails of the patient bed were found to have an accumulation of dust, hair, and a brown substance. The patient bed table was found to have a white substance on the top. Staff B and staff I confirmed the findings at approximately 1115 on April 29, 2014. On April 29, 2014 at approximately 1116 the patient bathroom in room 667 was observed to have dirty tiles around the bathroom sink with an accumulation of a white substance. Staff B and staff I confirmed the findings on April 29, 2014 at approximately 1118.
On April 29, 2014 at approximately 1120 in room 665 of the surgical specialty unit the counter top of the room was found to have a clear gel substance smeared across the top. The patient bed table was found to be partially cleaned with visible missed areas on the top. Staff B and staff I confirmed the findings at approximately 1122 on April 29, 2014.
On April 29, 2014 at approximately 1126 in room 664 of the surgical specialty unit the inside rails of the patient bed were found to have an accumulation of dust and the " evacu-sled " had an accumulation of dirt and food particles around the seams. Staff B and staff I confirmed the findings at approximately 1130 on April 29, 2014.
On April 29, 2014 at 1515 a tour of the nephrology floor was conducted. A patient ready bed was found in the hallway and inspected. Tape residue was found on the inside of the bedrails. Staff A and staff D confirmed the findings on April 29, 2014 at 1517.
On April 29, 2014 at 1520 patient room 225 was inspected. The patient bedside table was found to be dirty. The patient bathroom in room 225 was found to have dirty tiles around the sink area and around the back tiled area of the commode. On April 29, 2014 at approximately 1525 staff A and staff D confirmed the findings.
