Based on observation, interview, and document review, the hospital failed to ensure the pharmacy was directed by a registered pharmacist. The person responsible for pharmaceutical services was the Director of Pharmacy (DOP), who was not licensed in the state of California (CA), contrary to the hospital's job description for the DOP.

As a result, the hospital could not ensure pharmaceutical services were under competent supervision to ensure safe medication use.

Findings:

During a visit to the Pharmacy at the Downtown Campus on 7/12/18 at 9:05 A.M., Staff 1 was introduced as the DOP. Staff 1 stated she was responsible for directing pharmaceutical services at the hospital. Staff 1 also stated she was not licensed in the state of California as a pharmacist.

During a group interview with members of the hospital's multi-disciplinary Quality Management Committee on 7/12/18 at 3:05 P.M., Staff 1 introduced herself as the DOP at the hospital. The Committee members stated licensed staff should be licensed in the state they are practicing in.

According to the DOP's job description, "Job Requirements... Required License... Current CA Registered Pharmacist...."

According to the Department of Pharmacy's organizational chart, effective from 6/4/18, Staff 1 was the DOP.
