Based on documents review and staff interview, it was determined the facility failed to properly maintain electrical wiring and equipment as required as evidenced by the lack of documentation for the receptacles test. These deficiencies could affect all occupants of the facility in case of a fire or other emergency.

The findings include:

During the records review process of the facility between 10:00 a.m., and 3:00 p.m., on 05/07/2013 with the Assistant Maintenance Director, it was noticed there was no documentation for the performance of the receptacles test. The report submitted during survey was completed on 02/27/2012.

During the Staff interview between 10:01 a.m., and 3:01 p.m., on 05/07/2013, the Assistant Maintenance Director stated the receptacles test was performed with 100% completion on 02/27/2012. She also stated the facility started testing the receptacles in 01/2013 and have not completed it 100% as of the survey date.
