Based on observation and interview, the facility failed to ensure the environment of care for the acute behavioral health unit had heating, ventilation and cooling vents that resisted self-injury by strangulation. This had the potential to affect all present and future patients to that unit. The facility had a census of 139 patients.

Findings include:

On 09/23/15 at 2:00 PM a tour of the B Tower Building, acute behavioral health, was conducted with Staff A. Observation of patient room bathrooms in rooms 428, 429, 430, 431, 432, 433, 434, 435, 436, 437, 440, 447, 449, 450, 451, 452, 453, 454, 456 revealed the vents on the B-Tower building (Acute Behavioral Health)were replaced with new vents that had half inch square laminar squares and were mounted into the drywall with two screws. The observation could not determine whether they were ligature prevention.

On 09/23/15 at 3:55 PM in an interview, Staff A confirmed he/she did not have supporting documentation as whether the new vents were ligature preventing.
