Based on review of medical records, other documents and interview, it was determined that the facility did not adequately provide the complainant/patient a response which addressed all the components in the complaint.
This was noted in one (1) of twenty (20) grievance files reviewed. (File #1)

Findings include:

The complainant/patient filed a written complaint which was received by the facility on 5/4/15. The complaint had multiple issues: upon arrival to the ED he was not hooked up to cardiac monitoring until he went to 17 North, Pain medication was not given for back pain, antipsychotic medications were not given and on 4/28/15 on 19 South, he was sexually assaulted by a CO (Correction Officer) on the second shift. Complainant alleges the CO touched his private parts and made inappropriate comments.

The response from the facility dated 5/6/15 did not address the sexual allegation component of the complaint and failed to advise the patient/complainant who to contact if not satisfied with the result of the investigation.
This was brought to the attention of Staff #1.
