Based on review of documentation, interview and tour on March 29, 2022, it was determined that the facility failed to keep a sanitary environment in all areas of the hospital.

Findings were:

Facility policy entitled "Discharge Room Cleaning Procedure" stated in part "Using germicidal cleaner and a microfiber clean cloth, sanitize all patient contact surfaces, starting with the bed. Wipe the top and sides of the mattress. Beginning at the head of the bed, fold mattress in half and clean the underside of the mattress, springs and frame. Inspect mattress during cleaning. Repeat process with the foot of the bed. Clean the head of the bed, moving to the bed rails and bed controls, then the foot of the bed and opposite side bed rails. Using the bed controls, raise the head and foot of the bed. Clean the underside of the bed beginning with the springs and clean all underparts of the bed. Clean wheels, removing all strings and debris, using putty knife if necessary. Proceed to clean overbed table, bedside table, phone, chairs, low ledges and counter, light switches and doorknobs. As with the high dusting task, begin at the entranceway and work around the room in a circle. Use a new cloth whenever existing cloth is soiled. Do not reintroduce dirty cloth into clean germicide.

Damp wipe stains and spots from walls and other vertical surfaces."

Tour of 4 unoccupied bays of the Emergency Department on 3/29/22, revealed stained countertops with sticky tape residue in all rooms. 3 of 4 rooms had dust and debris under mattresses indicating that they were not thoroughly cleaned. Photographs sent to the state dated 10/11/21, reveal blood splatter on the walls and the electric cords attached to the blood pressure cuff saturated with dried blood.

The above findings were confirmed by the Safety Officer on 3/29/22.
