Based on observation the facility failed to ensure that the smoke doors were maintained in a manner to resist the passage of smoke.

Findings include:

On 03/11/14 at approximately 3:00 PM,the entrance doors between the corridor and the surgical intensive care unit did not close in a manner to resist the passage of smoke. A gap of approximately 1/4'' was noted at the meeting edges.

Based on staff interviews conducted from 03/10/14-03/12/14, the hospital failed to ensure that all staff members are familiar with fire safety procedures.

Findings include:

Four (4) out of six (6) employees interviewed for fire safety throughout the hospital did not know the location of the fire alarm and/or when and how to activate them. Staff members indicated that they would dial 5151 on the phone as notification in the event of fire emergency. Staff members were also not familiar with the emergency exits during fire emergency.

Based on observation, the hospital failed to ensure that all the devices and components of the fire alarm system were maintained in good repair.

Findings include:

During testing of the manual pull stations of the Lenox Hill Hospital on 03/12/14 at approximately 10:00 AM, it was noted that the manual pull station outside the Kitchen Stair C on Floor B did not function immediately upon activation. The pull station had to be pulled three times for the alarm system to be activated.
The Director Of Engineering stated that the manual pull station was an old one and had to be replaced.
