Tag No.: A0123

Based on facility provided documentation, patient record review and interview with the administrator, the facility failed to provide one of four patients (#1) with a written notice of its decision that identified the hospital contact person, the steps taken on behalf of the patient, the results of the grievance process and the date of completion.

The findings include:

A review of the facility grievance log revealed a grievance was made to the facility by Patient #1's family on 8/6/10. The grievance allegations included a lack of assessment, failure to provide care to the patient, failure to monitor the patient and to meet the needs of the patient. The facility talked with the family and an investigation was done regarding the patient's care. A review of the grievance investigation revealed that the patient was continuously monitored during the stay, that the patient was sent to a general medical floor, then to a unit with continuous monitoring and finally to ICU. The patient received a thorough work up for multiple conditions and the final analysis was the patient received the appropriate care. An interview with the administrator on October 15, 2010 at 4:30pm, revealed that the facility did not provide a written response to the grievance. He further noted that the policy is to respond in writing within seven days, unless the facility requests an extension, at which time the written response should be completed within sixty days. The facility had not provided a written response as of the day of the survey.
