Based on medical record review a registered nurse did not evaluate the nursing care provided to Patient #1 prior to discharge from the ED.
Findings include:
Review of Nursing Assessment dated 5/14/12 at 17:18 revealed Patient #1 arrived via EMS with unsteady gait and needed to be lifted to the cart. Patient incontinent of stool and urine.
Review of Nursing Note dated 5/15/12 at 9:43 revealed meal tray given to patient.
Review of Nursing Note dated 5/15/12 at 13:51 revealed patient discharged to home in a wheelchair.
Telephone interview on 7/24/12 with Staff #1 revealed that he drove the patient to her home and attempted to help her out of the van but the patient could not stand up alone. He placed the patient on the curb and called 911. The fire department responded.
There is no evidence from medical record review that a nursing assessment was performed prior to the patient's discharge on 5/15/12.
