DISCHARGE PLANNING EVALUATION

Based on interview, record review and review of the facility's policy and procedure the facility failed to ensure that discharge planning evaluation was initiated on 2 of the 4 patients identified as having a discharge, Patient #17 or transfer from the facility, Patient #18.

Findings:

Review of Patient #17 medical chart shows patient stayed in the facility 2 days from 11/21 to 11/23/2020 without having a discharge plan evaluation implemented for discharge.

Review of patient #18 medical chart shows patient stayed in the facility 2 days from 07/13 to 07/15/2020 without having a discharge plan evaluation implemented for transfer.

During an interview on 11/9/21 at 3:30 PM, with the Risk Manager stated, the Director of Nursing and myself review the patients charts for discharge planning and there is no documentation to show the patients were provided discharge planning. Discharge planning should begin upon the patient's admission to the facility.

A review of the Case Management policy and procedure 20-1-01 last revised 7/16/21 read, complete the nursing admission assessment and related documentation within 24 hours of admission. Discharge planning begins at the time of admission during the completing of the case management discharge planning assessment E-form.
