The Hospital reported a census of 4 patients with 34 records reviewed. Based on observation, policy review, manufacturer's guidelines reviewed, and staff interview, the infection control officer failed to develop an active infection control system to identify, report, investigate, monitor, and implement infection control practices for cleaning one of one Hydrocollators (device used to warm and store therapeutic packs in water) according to manufacturer ' s guidelines; and cleaning one of one patient chairs used for drawing blood samples in accordance with accepted professional principles.

Findings include:

- Manufacturer ' s guidelines of hydrocollator maintenance and cleaning reviewed on 9/24/13 at 2:30pm revealed " ...cleaned systematically at minimum intervals of every two weeks ... "

- Observation of physical therapy department on 9/24/13 at 2:30pm revealed one Hydrocollator. The Hydrocollator lacked documentation of cleaning and following manufacturer ' s guidelines for systematic cleaning.

- Staff B, physical therapist, interviewed on 9/24/13 at 2:30pm acknowledged the hospital failed to follow the manufacturer ' s guidelines for cleaning the Hydrocollator.

Centers for Disease Control (CDC) guidelines reviewed on 9/26/13 at 11:15am revealed " ...Standard Precautions ...cleaning and disinfecting of devices and environmental surfaces ...disinfect environmental surfaces and noncritical patient-care devices in between patient use if: There was direct contact to non-intact skin or mucous membrane or potential contamination with body fluids (e.g., blood, secretions) ... "

- Observation of laboratory department on 9/24/13 at 8:50am-9:20am revealed the chair all patients sit in to have blood work drawn used for five consecutive patients with one disinfectant cleaning between patient number two and patient number three.


Staff A, infection control officer, interviewed on 9/25/13 at 10:30 explained the hospital uses Association for Professionals in Infection Control (APIC) and CDC guidelines for infection control.
