Based on observations, the facility failed to maintain the integrity of smoke/fire barriers within the facility. Compromise of these barriers may allow smoke, toxins, and/or fire to easily travel from one smoke compartment to another potentially endangering everyone within the facility.
The findings:
During the Unit 31 facility tour with Maintenance Staff on 3/18/2014 beginning at approximately 1:00pm, penetrations were found in the smoke/fire barriers located in room 1229 (data cable) and room 1147 wall penetrations, missing J box covers. Holes/penetrations within fire/smoke rated partitions must be filled with the required rated caulk.
