Based on observations made during tour and staff interview, it was determined that the facility failed to maintain a fire barrier with at least a two hour fire resistance rating between the main residential building and the new addition. This could affect all 113 patients in the facility.

The facility has a capacity of 113 beds, with a census of 113 at the time of the survey.

Findings include:

Tour was conducted with Staff A and Staff E on 9/12/11 from 2:35 PM until 4:40 PM. During this tour the fire barrier was observed between the main residential building, which was a Type II(111) construction, and the new addition, which was a Type V(111) construction. On the north side of the fire barrier, in the corridor and in the admissions office, exposed wood including wooden joists could be seen where the drywall did not extend all the way to the roof decking. This finding was confirmed by Staff E during the tour and on 9/14/11 at 1:15 PM.

Based on observations made during tour and staff interview, it was determined that the facility failed to maintain a smoke barrier with at least a one half hour fire resistance rating and that did not reduce the original level of fire safety on the second floor of the residential building. This could affect all patients on the second floor of the building.

The facility has a capacity of 113 beds, with a census of 113 at the time of the survey.

Findings include:

Tour was conducted with Staff A and Staff E on 9/12/11 from 2:35 PM until 4:40 PM. The barrier on the second floor, midway down the residential corridor and near room 213, was originally built as a two hour fire wall, but was currently designated on facility floor plans as a smoke wall. This barrier was observed to have an unsealed opening through the wall that was approximately six inches by eight inches, through which black cable had been run. This finding was confirmed by Staff E during the tour.
