Based on a review of facility documents and staff interview (EMP), it was determined that the governing body failed to bear responsibility for contracted services.

Findings include:

Review of facility documentation "Performance Improvement Plan ... January 2016" revealed "In support of our mission and values, UPMC McKeesport will: ... 2. Develop and organization-wide performance improvement program that monitors and evaluates performance. 3. Identify trends within data and develop process improvement activities designed to improve care and service. ... A. The Board of Directors has the ultimate responsibility for performance improvement."

Review of facility documentation "Prestco Professional Services, LLC. ... a. Crawling & Flying Insect Control 1) Regular, periodic pest control treatments with EPA Registered insecticides and insects including cockroaches (all types), ants, spiders, centipedes, millipedes, sow bugs, silverfish, firebrats, stink bugs and other typical crawling pests and occasional invaders."

1. A request was made to review quality on the the above contract and none was provided.

Interview with EMP9 on May 25, 2016, at approximately 9:30 AM when asked if there was any quality done on the above contract service revealed "No"

FACILITIES, SUPPLIES, EQUIPMENT MAINTENANCE

Based on review of facility contract, tour of facility, and staff interviews (EMP), it was determined that the facility failed provide prevention of rodents, pests, and insects.

Review of Pest Control and Integrated Pest Management contract revealed, " ... 1) Pest control treatments shall be performed in, but not limited, the following areas: 1) (examples below) 1. Coffee stations, 2. Cafeteria, 3. Bio-waste area, 4. Generator room, 5. Meeting/board rooms, 6. Janitorial/mechanical closets, 7. Entrances/exits, 8. Negative pressure room, 9. Rest rooms, 10. Loading docks/dumpster areas, 11. Water meter room, 12. Warehouse areas, 13. HVAC/Storage room, 14. Outside break area ... Special Service Request Log Book 1) 'Special Service Request' log book will be made available in the Customer's location. The 'Request Log Book' consists of a special binder containing 'Special pest control Service Request' forms and MSDS sheets for all chemicals and products used for all chemicals and products used by the Contractor. Customer's management will be encouraged to indicate on these forms specific sightings of insect or rodent pests. Contractor's pest control operator will give priority to the 'special service requests' during each regularly scheduled service call."

Review of Weekly Pest Service Reports from December 11, 2015 to May 13, 2016, revealed the following on each weekly report. "Open Conditions: Interior: Condition: Doors need to be rodent proofed throughout, if light gets under door, rodents can get under door. Action: Repair or replace as needed."

Tour of facility on May 25, 2016, at 1:00 PM, revealed light showing under door on 1st floor Mansfield building main entrance, patio entrance, Kelly building main entrance, 2 Crawford main entrance, 2 Crawford employee entrance, 1 Crawford entrance, Subway restaurant entrance, gift shop back door, Emergency Department ambulance and patient entrances.

Interview with EMP30 on May 25, 2016 at 1:30 PM confirmed the above findings and revealed, "I didn't know anything about it (Pest Service Weekly Reports)."

Interview with EMP31 on May 26, 2016 at 11:00 AM when asked if any employees were treated for insect bites revealed, "We saw an employee last week, who had a bite on the neck."
