Based on interview and record review nursing staff failed to remove the IV catheter prior to the patient being discharged to home.
Findings:
On 12/31/2014 @ 1737 the patient was brought to the Emergency Room by ambulance for nausea and anxiety. The patient was seen by the physician and had an IV catheter for IV fluids and medication ordered . The nurse documented in the medical record that she started the IV, gave the IV fluids and gave the patient the medication for anxiety.
The patient remained in the Emergency Room until 01/01/2015 @ 1136 when she was discharged home with her son- in- law. The nurse did not document in the nurse's notes or discharge instructions that the IV catheter had been removed nor any after care instruction for the IV site after removal of the catheter. The patient was brought to another Emergency Room on 01/01/2015 @ 1844 for removal of the IV catheter.
An interview with the Emergency Room Nurse Manager on 02/05/2015 @ 1210 confirmed the event.
No Plan of Correction is required as the hospital has already identified the problem and implemented acceptable corrective measures.
