Sprinkler System - Maintenance and Testing

Based on observation and interview, it was determined the facility failed to maintain the automatic sprinkler system, in one instance, affecting one of ten smoke compartments within the component.
Findings include:
1. Observation on February 28, 2023, at 9:00 a.m., revealed there was multiple unsealed ceiling penetrations, in Room 180 on the 1st floor.
Interview with Facility Administrator and Maintenance Director on February 28, 2023, at 10:00 a.m., confirmed the automatic sprinkler system deficiency.
Electrical Systems - Receptacles

Based on observation and interview, it was determined the facility failed to maintain electrical receptacles in two instances, affecting two of ten smoke compartments within the component.
Findings include:
1. Observation on February 28, 2023, revealed there were electrical outlets, within six feet of a sink, which were not GFCI protected, in the following locations:
a) 9:07 a.m., in Room ISO 333;
b) 9:15 a.m., in Room ISO 354.
Interview with the Facility Administrator and Maintenance Director on February 28, 2023, at 10:00 a.m. confirmed the electrical receptacles were not GFCI protected.
