On facility tour between 09:00 AM and 12:30 PM on 01/25/2016, observation and documentation reviewed revealed that there was not a single plan for the out of service plan for the fire sprinkler system.


This deficient practice was confirmed by the Facility Maintenance Director at the time of discovery.
