Based on observation the entity failed to ensure the facility holds evacuation drills at least quarterly for each shift of personnel and under varied conditions to ensure that all personnel on all shifts are trained to perform assigned tasks;ensure that all personnel on all shifts are familiar with the use of the facility's emergency and disaster plans and procedures. The facility must file a report and evaluation on each drill.

It was observed at apporximately 12:15pm on 1-30-13, that the fire drills forms are not completely filled out. Creating a potential hazard for residents and staffs in the event of a fire.
