Based on observations the facility failed to provide doors with self-closing or automatic -closing in accordance with 19.2.2.2.6.
Finding include:
While inspecting smoke barrier doors on September 17, 2014 at 11:00 a.m., the maintenance supervisor and the surveyor observed following:
1. The smoke barrier doors near Room 101 did not close completely.
These deficient practices have the potential of affecting 2 of 3 smoke compartments.
The administrator and maintenance director were notified during the survey and in the exit conference.
