Based on observation two sets of rated corridor doors failed to latch closed when tested three of three times. Failing to ensure proper operation of rated corridor doors from heat or smoke could cause harm to the residents and/or staff during an emergency.
NFPA 101, Life Safety Code, 2012 edition, Chapter 19, Section 19.3.6.3.5. "Doors shall be provided with a means for keeping the door closed that is acceptable to the authority having jurisdiction."
Findings Include:
Observations made while on tour on April 14, 2021, revealed the rated corridor double doors in the Mesquite front hall and back hall failed to latch closed when tested three of three times. The doors in the Mesquite back hall were missing one one (1) latch which failed the door from latching. The Mesquite front hall doors were out of adjustment and would not latch.
During the exit conference on April 14, 2021 the above findings were again acknowledged by the management staff.

(1) Based on observation the facility failed to provide a protective guards on the light bulbs located in the basement mechanical room. Failure to keep light guards on the light bulbs could cause accidental damage or possibly a fire, which could cause harm to the patients and/or staff.
(2) Based on observation the facility failed to ensure electrical panels and electrical outlets were protected with covers. Failure to ensure protective covers are on electrical panels and outlets could cause harm and/or possibly a fire, which could cause harm to the patients and/or staff.
NFPA 101 Life Safety Code, 2012, Chapter 19, Section 19.5.1, "Utilities shall comply with the provisions of Section 9.1. Section 9.1.2, "Electrical wiring and equipment installed shall be in accordance with NFPA 70, 2011 Edition, "National Electrical Code." NEC, 2011, Article 110, Section 110-27 (b) Prevent Physical Damage."In locations where electric equipment is likely to be exposed to physical damage, enclosures or guards shall be so arranged and of such strength as to prevent such damage.
Findings include:
(1) Observations made while on tour on April 14, 2021, revealed several light bulbs were exposed basement mechanical room, where the main fire panel is. There was no protective bulb covers or protective wiring on the light fixtures.
(2) Observations made while on tour on April 14, 2021, revealed two (2) electrical panels were removed from the facility fire panel, exposing the internal wiring. Three (3) open electrical junction boxes were seen. One (1) electrical panel cover was removed from an air duct smoke detector, exposing the internal wiring. Several electrical outlets were seen without the protective cover plates.
During the exit conference on April 14, 2021, the above findings were again acknowledged by the management staff.
